Observation

There was a charger on the floor in the nap room that has a cord capapble of forming a loop and is accessible to the children - provider removed during visit. In the backyard the gutter spout is detached/broken from the gutter and it is accessible to the children. The new play structure has openings in the "wobbly steps" and the plastic "hole openings" that are entrampments. The holes measured 4in. wide and 6in in length. This does not meet There was an adapter/electrical cord with something plugged in it. They both had loose cables and it was not plugged in, they were accessible to the children - provider removed during visit.
